EF cold idle problem

I have a problem with a 1995 6 cyl EF (140000km) when cold. It starts then stalls due to insufficient revs. at idle. When I start it by holding accelerator down a bit it is ok, but stalls when you try to drive off or let it idle. By revving it at about 2000 rpm for about 30 secs it is then ok (or at least it runs, albeit roughly) If you stop the car for about 30 mins, then restart it, then this problem repets itself. The only difference being that it takes longer to idle normally when it is stone cold.
I have checked the base idle setting which is 100% ok. The car has had new leads & plugs, new air cleaner & fuel filter fitted.
Any help would be appreciated.
