Low stall torque converters have a tendency to cause that "jerk" when you put it in gear but if it's very harsh, I would tend to believe the TC is on its way out. The shuddering you're feeling at low rpm's is most likely caused by the torque converter lockup not working properly. A new converter is fairly inexpensive but it's a big job to change.
