'97 F-150/5.4; engine whines -HELP!!

I have 183,000 miles on my lariat and have only had to replace the #7 coil at 120k . Otherwise perfect truck. Except for this whining problem which started at about 135k (I had my engine steam cleaned at 125k) . It progressively has gotten worse until 170k .The noise is at idle and up to about 15-20 MPH . goes away at higher speeds. seems to get louder with AC on . Saw a mechanic and he thought it was the alternator. I've changed the belt once at 80k and never changed the tensioner. Any ideas ?
