replace clutch?or only an adjustment

hi everyone-
i have a 88 F150 4.9L 5SP W/ 118,000 MILES.lately its been hard to shift gears,i can shift but it doesnt shift easy like it used to,when shifting now i have to use a little force with the shifter.also,when driving forward and i come to a complete stop,if i try to put it in reverse,i get that grinding noise.clutch pedal DOESNT seem to grab at the bottom of the floor.is this only a matter of a simple adjustment?or does the clutch need replacemnt?btw,fluid level is ok.any info would be appreciated.

sounds to me like the clutch is on the way out and i would go for the replacement.
i always felt that 88 f150's were a little tall on gearing anyway, and if the gears are grinding on you it is time for a replacement as it will only get worse.
i might be wrong but i dont think they are adjustable.
