I've got several problem hinges on my vans. Rusting up is a natural hazard of any exposed hinge.
My mechanic drilled a small hole through the door half of the hinge just deep enough to expose the hinge pin. Then he sprayed penetrating oil with grease, not WD40, into the hole and opened and closed the door a few times, then repeated. Each time rust would come out and the door would open and close easier.
Alternatively, you could put some Zerk fittings in the hole (make sure the spot doesn't conflict with opening the door) and squirt grease in the hinge.
