Excursion 6.8L rough idle -need ideas

About 600 miles ago upon returning from a trip I noticed rough idle and hesitiation. Assuming that I picked up bad gas like last year I added STP products for water in fuel and to clean injectors. I also ran the tank down and filled with good high octane. I replaced the fuel filter too.

It seemed to help after 50 miles but reappered sporadically. I then went to a garage and had them clean the injectors. Didn't clear up right away but then was good for a hundred or so miles.

Yesterday it came back. Stumbles big time at around 1900 rpm especially up hill. At an idles it misses continually.

Any ideas what I can try next? It's intermitant so to me it seems like a fuel issue.

Replace (or clean) the idle air controller (IAC) and throttle position sensor (TPS). TPS is the most common but after so many miles the IAC valve will carbon up and start to cause problems. Don't forget to install new plugs as well for optimal performance

Don't forget to clean the MAF sensor.
