Removing by hand depends on alot of things. How strong your grip is, how much oil/grease is on the filter, how much hand room you have, and how tight the last monkey tightened it. All that aside, this is the type I've used for the last 30 years or so, and there has not been any filter that I couldn't gain access to. They make two sizes. http://www.drillspot.com/pimages/3165/316536_300.jpg
I use a rubber grip wrench. It works great for most applications. Every couple years I have to resort to taking a screw driver to it (my persuader, lol). Doing so you have to be careful not to tear the filter in half, or else you have a much bigger problem.
Any good large size strap wrench. As stated before, depending how tight it was last tightened, how greasy it is, and the strength of your grip, you may be able to get it off without the strap wrench. If it's super tight, get the strap wrench as close to the base of the filter as possible. If you don't, it will crush the filter rather than get it off. May still take muscle, but you will eventually get it off.
And make sure the old rubber gasket comes off with the old filter and is not stuck to the matting surface.
