Help

I was perfoming a self diagnostic test on my 1994 mustang gt and I am getting codes 181 and 189 haynes manual says Adaptive fuel rich limit reached at part throttle right side. and Adaptive fuel rich limit reached left side. Does anyone know what in the world this means.

What kind of up grades do you have on the motor ? Common problem is you up graded the exhaust and didn't use a cat eliminator or didn't hook the O2 back up. Mainly I need more information.
