My 91 4.0L 5 spd manual is acting up. When i let off the gas the truck makes a high pitch noise like gears not quite meshing and the truck does not slow down ( like nothing is holding it back). One point is that it does not happen all the time. Does this sound like transmission or transfer case?

It's more than likely the transmission.

The transfer case does rob the vehicle of some power, but it won't cut it on and off.

Curious though, but have you checked the fluid levels?

Thanks for the reply.
The fluid levels are good. I installed a used trans. hoping to solve the problem but it is still there. That is what makes me think transfer case. It is almost like it (trans. or trans. case ) slip into neutral when i let off the gas, then when i apply more gas it moves forward as normal.
