possible cooling fan problem?? any help greatly appreciated

i own a 1997 ford tbird lx it has the 4.6 v8 and it is currently having an overheating issue....The car will overheat to the point of coolant coming out of the reservoir tank and i cant figure out why,,, heres what i have recently done/replaced attempting to resolve the problem

1, replaced thermostat
2, replaced reservoir 16lb cap
3,flushed coolant
4, "supposedly has had water pump replaced before my purchase"

i seem to think that the rad fan isnt working correctly, the car will get very hot,, "temp gauge over 3/4 up" before the fan will even kick on them it will only stay on for a short period of time befre the engine even cools,, weird thing is that the fan will run constantly with a/c on

could the electric motor be faulty on my fan?? the motor shows no signs of a bad head gasket or intake gasket and i am stumped as to why the car is running so hot please help
