Engine code

I had 3 codes on my friends windstar. Bank one and bank 2. insufficient flow. I reset the codes, and what do you know, the light is back on. Threw my reader on. now its only one code. "P0401 exhaust gas recirculation. -insufficient flow detected." What is this caused from? i thought it was gas flow, gasoline, i read it wrong, so its exhaust gas going back into the engine?

That's the exhaust going back into the engine. It keeps the engine from pinging and cuts NOX.

so is it a sensor or the internal flange located in the Y-pipe? or blockage?

It could be any one of them. You can run vacuum to the valve and see if the engine runs rough or stalls when you do.
