Thinking about this from another angle, the transmission is only in second gear.
That narrows it down to some fault with either the second gear solenoid or the drive path through, in that gear.
My feeling is that it's program related and may be a TSB on the issue doing a PCM flash correction.
There are a lot of things that get into the action, from RPM, road speed, throttle position, load etc.
What driveline angle has to do with this is beyond me, for the specific fault.
