I work for a Navistar dealer, we see lots of T444E's and Powerstrokes. The only one I have seen with this type of damage
got a gulp of water and bent a rod, it eventualy broke.
I would be wondering if this had occured or if an injector was changed without draining the oil and fuel rails in the head. This could bend a rod when the engine is started as lots of oil will run into the cylinder. Also if the injector orings or the fuel pump fails allowing fuel dilution of the oil, and bearing failure. I have never seen any bottom end failures without some other cause on these engines.
