generic electric fan wiring

Hi folks. I have a 68 Mercury Montego with a replacement electric fan. The fan has a built in thermo and worked well until the motor was rebuilt and the fan was removed... Now I'm trying to figure out what wire goes where on the fan plug board. The board has five slots, + G M C B. There are three wires to feed, ground, 12v battery direct and on-switch from starter clinoid.

I can get the fan to run constantly by plugging ground to G, switch to M and Battery to B. How do I get it to run automatically using the built in thermo??? Thanks in advance...
