How to properly set timing for a 1993 E350, 7.5L, 460 this motor has "spout". At TDC, rotor at #1 in distributor, should spout be in at this time? I can't get it to turn over. If / when I get it started, warm it up 20, 30 min. turn off, remove spout, start engine, set with timing light 10° advance, turn engine off, tighten distributor bolt, start up again and check with timing light.
Is this correct or am I missing something? Please let me know exactly what I should be doing.

No. Remove the spout connector, set timing, then reinstall spout. The spout allows the PCM to set timing according to load/fuel conditions, when the engine is running, but the PCM needs to be disconnected when setting initial timing.
